Requesting Patient Medical Records

Center for Sports Medicine & Orthopaedics has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests.

You can request the release of your records by using the instructions below, or you may come into any of our locations to sign the request for the release. With either option, your medical record will be sent by HealthMark. Please note that the typical turnaround time for medical records release is 7 to 10 days.

Requests may be submitted electronically to HealthMark’s Request Manager at https://requestmanager.healthmark-group.com. Once logged in, select “Submit Request” from the menu options and enter all required fields to provide an authorization directly to HealthMark. Your medical record request will be processed and a notification will be sent via mail or email once complete and available for download.
