Requesting Patient Medical Records
- To get started, please click the link below. We kindly ask that you use this online option rather than making the request at one of our offices, as in-person submissions may delay processing.
- There is no charge for your medical records. There is a $15 fee for digital images if you need imaging – like MRI, X-Ray, or a CT Scan. Please use the same link below. Payment is required at the time of submission.
- Our standard turnaround time is 5–7 business days from the time of submission. Please note that medical records requests not submitted through this online process may take longer to complete.
