These forms are typically provided by your employer or disability carrier and are not available through this link.
- Our standard turnaround time is 5–7 business days from the time payment is received. Expedited processing is available for an additional fee. Please note that forms not submitted through this online process may take longer to complete.
- Payment is required at the time of submission. Fees are $30 per form, $15 for an updated form, and $50 for expedited processing, which includes a guaranteed 3-business-day turnaround.
- FMLA and Disability forms must be submitted separately and cannot be completed within the same request.
