We offer bundled payment options for those who have a high insurance deductible, out-of-network insurance, or no insurance coverage. Marketplace bundles help self-pay patients, self-insured employers, health sharing ministries and anyone who bears the out-of-pocket expense for services.

Please note that if you have insurance or are a member of a healthcare sharing ministry (HCSM), choosing one of these self-pay packages waives your right to send the charges associated with the appointment to insurance. CSMO cannot provide itemized charges associated with these packages.
